Client Services
Client services offered at the Coastal Bend AIDS Foundation is funded by five different grants which enable us to offer many services to our clients. Our goal is to work within the community to form alliances and partnerships with various agencies which help us provide the best care and treatment for individuals who are HIV positive or have been diagnosed with AIDS.
Case Management (CM) HIV/AIDS Services:
Funding for CM is provided by DSHS through an Administrative Agency – Bexar County Housing and Human Services (BCHHS) for Ryan White-Title II, State Health Social Services, and Housing Opportunities for Persons with AIDS (HOPWA) and through the University of Texas, Health Science Center – San Antonio for Ryan White Title IV services. The Case Management services delivered provide clients with necessary support services and financial assistance. Case Managers serve as “brokers” of services—linking clients to community resources and assisting them navigate social services systems and to increase access and utilization of resources. Financial Assistance is made available to eligible clients to subsidize income through rental assistance, emergency payments for utilities, dental services, medical co-payments, medication/prescriptions, counseling and mental health, health insurance, and transportation assistance. The primary goal of case management is to assist clients improve and maintain individual health through medication adherence and lifestyle management, development and maintenance of social support systems, and prevent transmission and/or re-infection of HIV. Our case management team is a group of caring and knowledgeable professionals who are trained to offer a comprehensive referral program which includes financial assistance with:
• Medications
• Physicians specializing in HIV/AIDS
• Outpatient ambulatory care services
• Insurance co-payments & premiums
• Emergency & long-term rental assistance
• Emergency utility assistance
HIV Early Intervention (HEI)
HEI is funded by the Department of State Health Services (DSHS). The HEI Program is designed to provide early intervention (intensive case management) with clients who are substance users/abusers with HIV/AIDS. The HEI program coordinates community continuity of care for clients with high level needs—assisting clients to navigate social services systems and to increase access and utilization of resources.
The primary goal of HEI is to move clients through the “stages of change” necessary to achieve and maintain drug free lifestyles/long-term recovery and improve client individual health as related to HIV/AIDS through medication adherence and the provision of routine medical and other support services.
HOPWA
Housing Opportunity for People With AIDS:
Housing Opportunity for People With AIDS funds a housing assistance program for income eligible families who have at least one member diagnosed with HIV/AIDS.
The goal is to prevent homelessness and to support independent living. The monies support two assistance options:
• Long – Term Assistance – which is a tenant-based program including rental assistance for shared housing arrangements. It helps families with the rent and utilities until there is no longer a need, or they are able to secure other affordable housing.
• Short-Term/Emergency Assistance – which is for rent, mortgage, and utility payments to prevent homelessness. This assistance is available for a period not to exceed 21 weeks in any 52-week period.
Ryan White Title IV
The Title IV program is specifically aimed at serving women, infants, and children who have either themselves been diagnosed as being HIV positive, or live in a household with another individual who has been diagnosed. Each family is offered the same benefits as other CBAF clients through this grant.
Individuals who are eligible for this program are placed with a specialized case manager who primarily works with this population and is trained to meet their specific needs.
