| The
Coastal Bend AIDS Foundation is a 501(c)(3),
non-profit organization, established on April
30, 1986, by community volunteers to provide support
for friends and family members infected or affected
by HIV/AIDS. CBAF serves twelve counties: Aransas,
Bee, Brooks, Duval, Jim Wells, Kenedy, Kleberg,
Live Oak, McMullen, Nueces, Refugio, and San Patricio.
CBAF
provides services to the community through case
management of nearly 400 HIV/AIDS clients on a
regular basis, plus three outreach projects: Project
CARE (Community Awareness Reinforcing Education),
which serves an average of 3,000 individuals yearly;
and Project FREE and Project BESOS which together
reach approximately 5,400 individuals annually.

Our vision
at CBAF is to enhance social services in the Coastal
Bend area through outreach, education, prevention
and intervention; actively encourage and support
mobilization through group and individual advocacy;
and leverage resources to support service delivery
systems to address community needs. Our primary
goal is to empower individuals and families to
take control of their lives through improved decision-making
skills and overall wellness and health promotion
initiatives.

CBAF
will achieve individual and community empowerment
through evidence-based strategies to provide
prevention, intervention, testing, and referrals
for client services and access into care for
medical, substance abuse, and mental health
services.
Through
community and resource mobilization, CBAF is
a front-runner, dedicated to the prevention
of HIV/AIDS, tobacco, alcohol and drug abuse
through our educational and outreach programs
in the community and schools. Through these
programs, those individuals most at risk—our
young people—are given information, testing,
counseling and life choices that help keep them
substance abuse and disease free.

Quality
Management
CBAF has a developed quality assurance program
that is comprised of the administrative staff
(ED, PD, HR, CFO), the (5) Program Coordinators,
and a Physician that serves CBAF clients. The
Performance Improvement Committee (PIC) ensures
cost and program effectiveness for service delivery.
The PIC meets quarterly to review program service
delivery goals, performance measures and outcomes,
and makes appropriate adjustments to for continuity
of care and cost containment.
Program
Management
Individual Program Coordinators (PC) are responsible
for day-to-day operation, monitoring, tracking
of service delivery goals, direct supervision,
training, and management for department staff
and development of program operating procedures.
